Supported Living for Persons With Disabilities

The agency supports affordable and equal housing opportunities through the Northwest Housing Initiative Program, funded by the Federal Housing and Urban Development (HUD ). The Enclaves is an independent living environment of single unit apartments located at 2250 Jammes Road, Jacksonville, Florida. Units are available for rent to qualified applicants. Points of Excellent Awards Banquet

The agency presented its 3nd Annual Points of Excellent Awards Banquet on Saturday,February 10th, 2007 at the Jacksonville Hyatt Regency Riverfront Hotel.

Jazz Social - 6PM / Dinner - 7PM

This celebratory event will feature the exquisite voice of Jazz, Gospel and R&B Recording Artist, Oleta Adams.

We will also recognize individual leaders for their outstanding service and contributions in the areas of healthcare, education, economic development, faith leadership and public service. A special award named in honor of long time community servant, Sara Cotton, will be presented.

Other activities include a VIP Receptionand Silent Auction beginning at 6:00 PM

Individual tickets are $75.00, Corporate Sponsors are $1,000.00, Gold Sponsors are $2,000.00 and Platinum Sponsors are $3,000.00. All proceeds will benefit Northwest Behavioral Health programs.
